About the Saks Fifth Avenue Receipt Template
A department-store receipt is the printed record of a purchase at a large retailer — the items with their tax lines, the subtotal and tax, the total, the card authorization, and the survey and thank-you lines. This template lays out that department-store format section by section, and you fill it in with your own store, items, and amounts. It is handy for keeping an expense record, reproducing a slip that has faded, building a mockup for a retailer's point-of-sale screen, or generating sample data for testing. Every value shown below is an editable example, not a real transaction.
Store header
The receipt opens with the store logo over its address — for example Saks Fifth Avenue, 5175 Westheimer Road, Houston, TX 77056, and a phone line. Replace these with your own store details.
Manager and register line
A short block names the manager and associate and stamps the register codes — STORE MGR: John Doe, ASSOC: 0304455 Joileen, ST:435 TRM:231 TRN:24353 TAKE. Each field is yours to set.
Item lines
Items print in an item, ID, and total layout, each spanning a few rows — a product name, its SKU and a style code, a unit-price line, and its own item-tax line. The sample lists SUTTON PLACE (1 @ 210.00, Item Tax 23.17 T) and NY DAYS (1 @ 320.00, Item Tax 31.12 T). Add as many items as the sale needs.
Totals
The totals block stacks the subtotal, the total tax, and the total, then the tender — Subtotal 530.00, Total Tax 54.29, TOTAL 584.29, Debit 584.29. Adjust the figures and tax to match the sale you are documenting.
Card authorization
A short paired-column block records the card and approval — a masked account number, an approval status, and an auth number, such as ACCT # /C xxxxxxxxxxxx3434, Apprvd-00, AUTH: 63455. Switch the payment to cash and this block simply drops away.
Survey and footer
The foot of the receipt carries the lines a department store usually prints: a store-survey prompt with a web address, an items-purchased count, and a thank-you block inviting the shopper to visit the site or download the app. Any of these can be edited or removed.
